Your Appointment

Lockhart Matter Dermatology sees patients from Plano, Richardson, Garland, McKinney, Allen, Frisco and Dallas. We are so excited to now have our office in Prosper. We invite you to take your time and tour our website as it contains many informational aspects that will be of interest to you and your care. When possible, we have included forms and information about insurance and the types of care we provide, all in an effort to make your visit and care nothing short of exceptional.

Because treatment cannot be provided without consent, all minors must be accompanied by a parent or legal guardian to sign the appropriate forms. The consent form for minors can be found by clicking the Patient Forms tab above. A minor is anyone under the age of 18.

All Patients

Please arrive 15 minutes before your appointment time. At your initial visit, you will need to allow enough time to check in, have your ID scanned into the system and give our personnel time to enter your information. You will also need to bring the following to your visit. 

• Completed Patient Portal (All medical history must be completed electronically before visit. If you are having any issues accessing your portal, please click here, see help below)
• Prescription history including oral and topical prescriptions, over the counter mediations and supplements
• Referrals and authorizations, if required by your insurance plan
• Insurance Cards
• Driver’s license

You will receive an email with automatic to access the Patient Portal for the first time where you will set up your own password. If you do not access the portal within 48 hours of receiving this email, the password will expire. You can simply click reset/forgot password, and the system will send you another password to access the system. If you get “locked out” please call the office (469-769-3376)to unlock your account. If more than one family member use the same email (ex: minors and parents or husband/wife), you must contact the office to get assistance setting up different usernames for portal access. Insurance

Before coming to Lockhart Matter Dermatology for your appointment, check with your insurance provider to see what is covered under your plan for medical and dermatology services. All patients utilizing their insurance coverage are required to bring their current insurance cards. Additionally, in order to be treated, you must present you or your Guarantor’s driver’s license or photo ID. If you have a copay, payment is to be made at time of service and may be made by cash, check or credit card.
